CHISEL (TROG 09.02)
CHISEL is the randomised trial that proved stereotactic radiotherapy beats conventional radiotherapy for early lung cancer in patients who cannot have surgery, with better local control and longer survival.
Overview
CHISEL randomised 101 patients with peripheral stage I non-small-cell lung cancer unfit for surgery to stereotactic ablative radiotherapy (54 Gy in three or 48 Gy in four fractions) or conventional radiotherapy (66 Gy in 33 or 50 Gy in 20). Local failure was less frequent with stereotactic treatment (hazard ratio 0.32) and overall survival was longer, median 5 years versus 3 years, with similar toxicity (Ball and colleagues, Lancet Oncology 2019).
