The trial that added toripalimab to chemotherapy for oesophageal cancer in China and showed a survival gain regardless of PD-L1 level.
Overview
514 patients. Median overall survival 17.0 versus 11.0 months (hazard ratio 0.58) and progression-free survival 5.7 versus 5.5 months (hazard ratio 0.58) at the interim analysis, with benefit independent of PD-L1 expression; published in Cancer Cell 2022. NMPA approval May 2022. One of five Chinese PD-1 trials (with ESCORT-1st, ORIENT-15, RATIONALE-306 and KEYNOTE-590) that made chemo-immunotherapy the first-line standard in a cancer where China has about half the world's cases.
- Median 17 vs 11 months with Toripalimab + TP compared with Placebo + TP; about 6 months longer for half the group.
- A median is a midpoint: half the people did better than this and half did worse, so it is not a prediction for any one person.
- Put another way, the treated group had about 42 percent lower chance of the event at any given time (hazard ratio 0.58).
- Overall survival counts deaths from any cause, so it is the most direct measure of whether a treatment helps people live longer.
- These results apply to the people the trial enrolled: First-line advanced oesophageal squamous cell carcinoma, China: toripalimab or placebo plus paclitaxel and cisplatin. People in a different situation may not see the same effect.
