ORIENT-32
A Chinese trial showing that a PD-1 antibody plus a bevacizumab copy extends life in liver cancer, mirroring the global IMbrave150 result in a hepatitis B population.
Overview
571 patients randomised 2:1. Overall survival hazard ratio 0.57 (median not reached versus 10.4 months) and progression-free survival hazard ratio 0.56 at the first interim analysis; about 94% of patients had hepatitis B, the cause of most Chinese liver cancer. NMPA approval June 2021. With IMbrave150 it established anti-PD-(L)1 plus anti-VEGF as the first-line standard and gave China a reimbursed domestic option.
- Median 10.4 months with Sorafenib.
- Sintilimab + IBI305: Median not reached at interim analysis.
- "Not reached" means that, when the data were analysed, more than half of that group had not yet had the event, which is good news for that group.
- A median is a midpoint: half the people did better than this and half did worse.
- Put another way, the treated group had about 43 percent lower chance of the event at any given time (hazard ratio 0.57).
- Overall survival counts deaths from any cause, so it is the most direct measure of whether a treatment helps people live longer.
- These results apply to the people the trial enrolled: First-line unresectable hepatocellular carcinoma, China (mostly hepatitis B-related): sintilimab plus the bevacizumab biosimilar IBI305 vs sorafenib. People in a different situation may not see the same effect.
- Median not reached at interim analysis.
