ZGDH3 (donafenib vs sorafenib)
The first trial in which a new pill beat sorafenib on survival in liver cancer, with a Chinese deuterated redesign of sorafenib itself.
Overview
668 patients, open label. Median overall survival 12.1 versus 10.3 months (hazard ratio 0.83) with fewer grade 3 or higher adverse events; published in the Journal of Clinical Oncology 2021. NMPA approval June 2021. A small gain, but the first head-to-head overall survival win against sorafenib and an example of incremental medicinal chemistry rewarded in China's market.
- Median 12.1 vs 10.3 months with Donafenib compared with Sorafenib; about 1.8 months longer for half the group.
- A median is a midpoint: half the people did better than this and half did worse, so it is not a prediction for any one person.
- Put another way, the treated group had about 17 percent lower chance of the event at any given time (hazard ratio 0.83).
- Overall survival counts deaths from any cause, so it is the most direct measure of whether a treatment helps people live longer.
- These results apply to the people the trial enrolled: First-line unresectable or metastatic hepatocellular carcinoma, China: donafenib vs sorafenib. People in a different situation may not see the same effect.
