Showed that women whose breast cancer looked high-risk to doctors but low-risk on the 70-gene test could skip chemotherapy with very little loss.
Overview
Among 6,693 women, 1,550 had high clinical risk but low genomic risk by MammaPrint; those randomised to no chemotherapy had 5-year distant metastasis-free survival of 94.7% (NEJM 2016), meeting the prespecified threshold, with a 1.5 percentage point difference versus chemotherapy that was not significant. The 8-year update (Lancet Oncology 2021) confirmed the finding, with a suggestion of chemotherapy benefit in women aged 50 or under. About 46% of clinically high-risk women could avoid chemotherapy.
- 94.7 out of 100 people reached this endpoint at 5 years with No chemotherapy (genomic risk).
- There was no comparison group, so this number cannot tell you how much of the effect is due to the treatment.
- This is a surrogate endpoint: it measures the cancer being controlled or absent on scans and tests, which often, but not always, translates into living longer.
- These results apply to the people the trial enrolled: Early breast cancer with discordant clinical and genomic (MammaPrint) risk: chemotherapy or not according to genomic risk. People in a different situation may not see the same effect.
