SJMB12
SJMB12 is the St. Jude trial that first treated medulloblastoma by its molecular type: children with the low-risk WNT form receive a much lower dose of radiotherapy to the brain and spine, those with the SHH form add the targeted drug vismodegib, and the others add two extra chemotherapy drugs; it is still running and the corpus records the design.
Overview
SJMB12 is a non-randomised phase 2 trial at St. Jude Children's Research Hospital and partner centres in which newly diagnosed medulloblastoma is assigned to strata by molecular subgroup (WNT, SHH, group 3 and 4) and clinical risk. Low-risk WNT patients receive craniospinal irradiation of 15 Gy with a reduced boost and four cycles of cyclophosphamide, cisplatin and vincristine; skeletally mature SHH patients add maintenance vismodegib; intermediate- and high-risk non-WNT, non-SHH patients add pemetrexed and gemcitabine. Randomised sub-studies test aerobic training and cognitive remediation. The primary endpoints are progression-free survival at two years in the low-risk WNT, standard-risk SHH and standard-risk group 3 and 4 strata.
The registry lists 660 participants with a primary completion date of October 2028 and no posted results. The corpus's WNT medulloblastoma page cites SJMB12 for 15 Gy craniospinal radiotherapy alongside the COG ACNS1422 study of 18 Gy.
