Overview
Peter Sasieni led the 2021 Lancet analysis showing the English HPV vaccination programme cut cervical cancer by 87% in women vaccinated at 12-13, one of the clearest demonstrations of cancer prevention ever recorded, and the modelling that underpins HPV-based and extended-interval cervical screening. He co-leads the NHS-Galleri trial of multi-cancer early detection and directs the Cancer Prevention Trials Unit.
NHS-Galleri is the trial that will decide whether a blood test for many cancers at once should be offered by a health system. Its endpoint is a reduction in late-stage cancer rather than deaths, so even a positive result leaves the mortality question to be settled by longer follow-up.
School-based vaccination at 12-13 with high uptake nearly abolishes cervical cancer in vaccinated cohorts, even with a vaccine covering only two HPV types. Screening intervals and the future of cervical screening can now be redesigned around vaccination status.
