The 2021 FIGO staging of vulvar cancer is simpler than before and was the first FIGO gynaecological staging derived from an analysis of patient data.
FIGO's committee worked with the US National Cancer Database, analysing 12,063 cases treated from 2010 to 2017 and grouping tumour characteristics with similar survival (Olawaiye and colleagues, International Journal of Gynecology and Obstetrics 2021). The result has two substages in stage I, none in stage II, three in stage III and two in stage IV; a new definition of depth of invasion; the same definition of lymph node metastasis as cervical cancer; and allows cross-sectional imaging findings. The AJCC lists its vulva chapter as a Version 9 protocol for 2024.
Most high-grade ovarian cancers begin at the tip of the fallopian tube; endometrial cancer lines the uterus, cervical cancer starts at the transformation zone; each drains to a different node group.
