Pancreatic neuroendocrine tumours have their own TNM staging, separate from ordinary pancreatic cancer, in which small tumours confined to the pancreas are stage I.
AJCC chapter 34 covers neuroendocrine tumours of the pancreas; the AJCC 2026 table lists a Version 9 protocol for 2024, one of six neuroendocrine sites updated that year. In the 8th edition tables reproduced by the NCI, stage I is a tumour limited to the pancreas and smaller than 2 cm with no node or distant spread; stage II is a tumour limited to the pancreas of 2 to 4 cm, or larger than 4 cm or invading the duodenum or bile duct, without spread. Grade by Ki-67 is reported separately and matters as much for treatment.
Most pancreatic cancers arise in the head next to the bile duct, which is why jaundice is the presenting sign; bile duct cancers are named by where along the tree they sit.
