Giving the heated abdominal wash as insurance, before any peritoneal disease has appeared, did not prevent it.
COLOPEC randomised 204 patients with clinical or pathological T4 or perforated colon cancer at nine Dutch HIPEC centres to adjuvant oxaliplatin HIPEC (460 mg/m2 for 30 minutes at 42 degrees, simultaneously or within five to eight weeks of resection) followed by routine adjuvant chemotherapy, or to adjuvant chemotherapy alone, with diagnostic laparoscopy at 18 months in everyone free of recurrence. Peritoneal metastases were diagnosed in 19 of 100 in the HIPEC group and 23 of 102 in the control group, and peritoneal metastasis-free survival at 18 months was 80.9 against 76.2 percent (one-sided p=0.28). Twelve of 87 patients who received HIPEC had postoperative complications and one developed encapsulating peritoneal sclerosis. Adjuvant HIPEC is not recommended on this evidence.
