Reopening the abdomen to look for and wash out invisible disease did not help, and one patient in four had a serious complication.
PROPHYLOCHIP randomised 150 patients at 23 French hospitals who were at high risk of colorectal peritoneal metastases (resected synchronous localised peritoneal metastases, resected ovarian metastases or a perforated tumour) and who were disease-free after six months of adjuvant chemotherapy, to systematic second-look surgery with oxaliplatin HIPEC or to surveillance. After a median 50.8 months, three-year disease-free survival was 53 percent with surveillance against 44 percent with second-look surgery (hazard ratio 0.97, 95% CI 0.61 to 1.56). Twenty-nine of 71 patients in the surgery group (41 percent) had grade 3 to 4 complications. Standard surveillance is enough.
