The small Chinese trial whose startling response rates at ASCO 2017 drew Johnson and Johnson to Legend Biotech and became the myeloma CAR-T Carvykti.
Overview
57 patients treated at Xi'an Jiaotong University's Second Affiliated Hospital (Journal of Hematology and Oncology 2018): objective response 88%, complete response 68%, median progression-free survival 15 months, manageable cytokine release syndrome; a further 17 patients were treated at three other sites. Presented at ASCO 2017 by Fan Xiaohu and Zhao Wanhong, it led to the December 2017 Janssen collaboration ($350 million upfront) and to CARTITUDE-1, FDA approval as ciltacabtagene autoleucel in 2022 and NMPA approval in 2024. The clearest case of a Chinese academic-industry cell therapy becoming a global standard.
- 88 out of 100 people had their tumour shrink with LCAR-B38M.
- There was no comparison group, so this number cannot tell you how much of the effect is due to the treatment.
- A response rate counts how many people had their tumours shrink by a set amount; shrinking is encouraging but does not by itself show that people live longer or feel better.
- These results apply to the people the trial enrolled: Relapsed or refractory multiple myeloma, four hospitals in China: LCAR-B38M, a BCMA CAR-T with two llama-derived binding domains. People in a different situation may not see the same effect.
- The trial selected people by a biomarker (BCMA); the result should not be assumed for people whose cancer does not have it.
- Only 57 people took part, so the numbers are less certain than in a large trial.
