POP-RT
POP-RT, from Mumbai, showed that treating the pelvic lymph nodes as well as the prostate improves disease control in high-risk prostate cancer, reviving a question earlier trials had left unanswered.
Overview
POP-RT randomised 224 men with high-risk, node-negative prostate cancer (most staged with PSMA PET) to image-guided prostate-only or whole-pelvic radiotherapy with two years of androgen deprivation. Five-year biochemical failure-free survival was 95 percent with pelvic radiotherapy and 81.2 percent with prostate-only treatment (hazard ratio 0.23), with better disease-free survival and more late urinary toxicity (Murthy and colleagues, Journal of Clinical Oncology 2021).
- 95 vs 81.2 out of 100 alive at 5 years with Whole-pelvic radiotherapy compared with Prostate-only radiotherapy; 13.8 more per 100.
- Roughly one extra person helped for every 7 treated. That is a rough figure taken from the two percentages, not a guarantee for any one person.
- Overall survival counts deaths from any cause, so it is the most direct measure of whether a treatment helps people live longer.
- These results apply to the people the trial enrolled: High-risk node-negative prostate cancer: prostate-only versus whole-pelvic radiotherapy, both with hormone therapy. People in a different situation may not see the same effect.
