London radiation oncologist who led ALSYMPCA, the trial that showed radium-223 lengthens survival in prostate cancer that has spread to bone, and RADICALS-RT, which showed radiotherapy after prostatectomy can safely wait until the PSA rises.
Overview
Chris Parker is a clinical oncologist at The Royal Marsden and The Institute of Cancer Research. He was first author of the 2013 New England Journal of Medicine report of ALSYMPCA, in which the alpha-emitter radium-223 improved overall survival in castration-resistant prostate cancer with bone metastases, and of the 2020 Lancet report of RADICALS-RT, which found that early salvage radiotherapy on PSA rise was as good as adjuvant radiotherapy after prostatectomy, with less toxicity.
Observation with early salvage radiotherapy at PSA recurrence is standard after prostatectomy, avoiding radiotherapy in most men who would never have needed it.
Radium-223 is an option for symptomatic bone-predominant castration-resistant prostate cancer, now less used since lutetium-PSMA, and should not be combined with abiraterone after the ERA 223 fracture signal.
