RECIP judges response on PSMA PET scans by the change in the total volume of PSMA-positive tumour and the appearance of new spots, and was built in men treated with lutetium-PSMA.
Gafita and colleagues (Journal of Nuclear Medicine 2022) studied 124 men with metastatic castration-resistant prostate cancer treated with lutetium-177 PSMA who had PSMA PET at baseline and 12 weeks. RECIP 1.0 defines complete response as no PSMA uptake; partial response as a fall of 30% or more in PSMA-positive tumour volume without new lesions; progression as a rise of 20% or more with new lesions; and stable disease as anything else. Median survival was 8.3 months with RECIP progression against 13.1 months with stable disease. A composite with PSA (PSA + RECIP) is proposed as a trial endpoint.
