PROMISE V2 is a standard way to read and report PSMA PET scans in prostate cancer, describing where the cancer is with a molecular-imaging version of TNM (miTNM) and tracking change in trials.
The prostate cancer molecular imaging standardized evaluation (PROMISE) criteria describe disease extent on PSMA PET as molecular imaging TNM. PROMISE V2 (Seifert and colleagues, European Urology 2023) updates miTNM, improves assessment of local disease, slightly modifies the PSMA-expression score, and adds a framework of parameters to record over time in clinical trials, which can feed PSMA PET progression (PPP) or RECIP. A low PSMA-expression score predicted shorter survival after lutetium-177 PSMA therapy in the studies reviewed.
