PERCIST judges response by how much sugar a tumour takes up on a PET scan rather than by its size, with a fall of 30% or more counting as a response.
Wahl and colleagues (Journal of Nuclear Medicine 2009) reviewed the limits of size-based criteria, which change little in effective treatment of lymphoma, sarcoma, liver cancer, mesothelioma and GIST, and proposed PERCIST 1.0. It measures lean-body-mass standardised uptake value in a fixed small region over the most active tumour, compares it with normal liver, treats the change as a continuous measure, requires a fall of 30% or more for response, and defers to RECIST 1.1 for tumours that do not take up FDG.
