Bendamustine
An East German chemotherapy rediscovered in the 2000s that became the backbone partner for rituximab in follicular, mantle cell and Waldenström lymphomas.
Approved 2008 for CLL and rituximab-refractory indolent NHL. StiL and BRIGHT trials showed bendamustine-rituximab at least as effective as R-CHOP with less alopecia and neuropathy, making BR the most common first-line indolent lymphoma regimen. Prolonged lymphopenia and infections (PJP, CMV) are the main concerns; combined with obinutuzumab (GALLIUM) or acalabrutinib (ECHO).

Given by infusion or injection in a clinic or hospital outpatient department, so it is a Part B drug: Medicare pays 80% after the Part B deductible and the patient owes 20% coinsurance, uncapped in Original Medicare unless a Medigap policy applies. Bendeka and Belrapzo are branded; generic bendamustine is available.
