Alabama's only NCI-designated comprehensive cancer centre, one of the original 1971 designees, serving the Deep South and studying cancer disparities.

One of the first cancer centres designated under the 1971 National Cancer Act, O'Neal is the only NCI comprehensive centre in Alabama and a reference centre for Mississippi and the Deep South. Research strengths include gynaecologic oncology and ovarian cancer, pancreatic cancer, neuro-oncology and head and neck cancer, with a long-running focus on rural and Black population health disparities. The UAB Proton Center, which opened in 2020, added proton beam therapy to the state. Selwyn Vickers, now MSK's president, was previously UAB's dean and a pancreatic surgeon here. Patients have access to phase 1 through phase 3 trials via UAB's Cancer Clinical Trials Office.
