Bicalutamide
The old antiandrogen pill used to block the testosterone flare from GnRH agonists and in combined androgen blockade; superseded by enzalutamide-class drugs.
Approved 1995 with LHRH agonist for metastatic prostate cancer; high-dose monotherapy (150 mg) in EU for locally advanced disease. Antiandrogen withdrawal response reflects its partial agonism. Still used for flare prevention and in low-resource settings.
